Quest for the Card Sweepstakes

OFFICIAL RULES NO PURCHASE NECESSARY.OPEN ONLY TO LEGAL RESIDENTS OF U.S., D.C. AND CANADA (EXCLUDING QUEBEC) 18 YEARS OF AGE OR OLDER AS OF SEPTEMBER 18, 2003. VOID IN PUERTO RICO, QUEBEC (CANADA) AND WHERE PROHIBITED.

1. Description: The Quest for the Card Sweepstakes (“Sweepstakes”) begins on September 18, 2003 at 5:00 p.m. ET and ends on November 3, 2003 at 12:00 p.m. ET (“Sweepstakes Period”). Sweepstakes Period is defined by Eastern Time (“ET”).

2. How to Enter: To participate, watch the Golf Channel nightly during the start dates and times specified in the Chart to obtain the Sweepstakes Code (“Code”). Check local TV listings for show schedules in your local viewing area, which may be changed or pre-empted without notice or call local station broadcasting program for details. Then, access the web site at http://www.TheGolfChannel.com (“Web Site”) during the Sweepstakes Period and click on the sweepstakes text link or icon. You must be registered with TheGolfChannel.com to enter the sweepstakes online. If you are a registered user, input your email address and Code (where indicated), then click “Enter” to enter sweepstakes. Entry into the Sweepstakes via the Internet is dependent on having the correct Code. If you are not a registered user of TheGolfChannel.com, after you input your email address and click “Enter”, you will be taken to TheGolfChannel.com sign up form. The sign up form requires your name, address (including zip code), birth date, telephone number (including area code), email address, and password of your choice (collectively “Entry Information”). Submit the completed sign up form and click “Sign-Up” to enter the sweepstakes. You will receive one (1) entry into the Sweepstakes when you enter as a current registrant or register and enter as a new registrant with the particular Code for that corresponding day. Participants can only register and enter the sweepstakes under one (1) user password and one (1) email address. Once an email address has been registered by a participant that email address and corresponding user ID cannot be used by any other person at TheGolfChannel.com Web Site. Only the participant registered under a particular email address with TheGolfChannel.com may participate in the Sweepstakes.

Limit one (1) Entry per person/day during the specified tournament dates/times listed in the above Event Chart. Entries must be received by November 3, 2003. Multiple Entries in excess of stated limits by the same person will be disqualified. All Entry Information is required and must be completed in full. If an entrant resides in a jurisdiction that deems him/her to be a minor, he/she must obtain parental/legal guardian consent prior to entering or submitting any personally identifiable information. Proof of delivery or receipt of Entry will not be deemed by Sponsor (defined below) as proof of entry. Any attempted form of entry other than described above is void

3. Eligibility: Sweepstakes is only open to legal residents of the fifty (50) states of the United States (including the District of Columbia, excluding Puerto Rico) and Canada (excluding Quebec), who are physically located and residing therein and who are 18 years of age or older as of September 18, 2003. Residents of Puerto Rico and Quebec are not eligible to enter. Employees of The Golf Channel, Inc. (“Sponsor”), PGA TOUR Employees, Skycastle Entertainment, Inc., Sullivan Compliance Company (“Sweepstakes Administrator”), their advertising or promotion agencies, those involved in the production, development, implementation or handling of this Sweepstakes, any agents acting for, or on behalf of the above entities, their respective parent companies, officers, directors, subsidiaries, affiliates, service providers, prize suppliers, or any other person or entity associated with this sweepstakes (collectively “Sweepstakes Entities”) and members of the immediate families (spouses, children, siblings, parents) and/or persons living in the same household as such persons, (whether or not related) are ineligible to enter Sweepstakes.

4. Description of Prizes/Odds of Winning: One (1) Grand Prize: One (1) trip for winner and one (1) guest for four (4) days/three (3) nights to the PGA Tour Golf Academy at The World Golf Village in Saint Augustine, FL. Trip package includes round trip coach air transportation for winner and guest from a major commercial airport near the winner’s residence (as determined by Sponsor in its sole discretion) in the U.S. or Canada to Saint Augustine, FL; standard hotel accommodations for three (3) nights (single room/double occupancy), three (3) days of golf instruction and three (3) days of golf following each day of golf instruction with on-the-course training, and a tour for winner and guest of the World Golf Hall of Fame (Approximate Retail Value (“ARV”) of Grand Prize is $4,000 U.S.). Three (3) First Prizes: One (1) framed autographed commemorative Nationwide Tour pin flag. The pin flag will be autographed by the top twenty 2003 Nationwide Tour Players who have earned their cards (ARV: $100 U.S. each). Ten (10) Second Prizes: One (1) Nationwide Tour hat (ARV: $20 U.S. each) Total ARV of all prizes $4,500 U.S. Limit one (1) Prize per person/family/household for the entire Sweepstakes Period. Odds of winning a Prize depends on the number of eligible entries received [with correct Codes]. Air transportation will be provided only if Grand Prize winner resides more than one hundred (100) miles from destination. Actual ARV of Grand Prize depends on point of departure and any airfare fluctuations. Trip is subject to air travel and hotel availability, holiday and black-out dates, and all other prize and travel restrictions. Grand Prize winner and guest must take air travel on same travel itinerary. Grand Prize winner is solely responsible for all costs, incidentals, and expenses not specifically mentioned as being included, including but not limited to, ground transportation, phone calls, unspecified meals and gratuities. Grand Prize winner and/or travel guest, if residents of Canada, must obtain all necessary and required travel documents (passport, etc.). Once determined, no change, extension or substitution of trip dates is permitted, except by Sponsor at its sole discretion. If Grand Prize winner is a resident of a jurisdiction that deems him/her to be a minor, he/she must be accompanied by his/her parent/legal guardian, and such person must pay his/her own expenses unless deemed to be the travel guest. If travel guest is a resident of a jurisdiction that deems him/her to be a minor, he/she must be accompanied by his/her parent/legal guardian, and such person must pay his/her own expenses unless deemed to be the winner. Grand Prize winner will have one (1) year from first date of notification of winning to take trip. All winners are solely responsible for all costs, incidentals, and any other expenses not specifically mentioned as being included. No substitution, transfer or cash equivalent for a prize (or prize component), except that Sponsor, at its sole discretion, may substitute a prize (or prize component) of equal value for any reason. Any depiction of prizes is for illustrative purposes only. Prizes and prize notifications shipped to U.S. and Canadian (Quebec excluded) addresses only. All federal, state, provincial, or other taxes are sole responsibility of winners.

5. Drawing and Notification: Potential winners will be randomly selected by the Sponsor from all eligible entries received in a drawing held on or about November 3, 2003. Potential winners (their parent or legal guardian, if winners are residents of a jurisdiction that does not deem them to be of the age of majority) may be notified by phone, mail and/or email within approximately three (3) days of the Drawing Dates. Canadian residents must correctly answer a time based mathematical skill testing question administered by phone/mail to be eligible for prize. If a potential Prize winner cannot be reached after a reasonable effort has been exerted, if he/she is found to be ineligible, if his/her prize notification is returned as undeliverable, or if he/she cannot or does not comply with these Official Rules, that potential winner will be disqualified and an alternate potential winner may be selected at Sponsor’s sole discretion. If notified by email, potential winners will have one (1) business day within which to respond to the prize notification email, or be disqualified and their prizes will be forfeited and alternate potential winners may be selected. If an entrant changes his/her email address after he/she enters the Sweepstakes, it is his/her sole responsibility to notify Sponsor by postal mailing notice to the Sweepstakes Administrator address (listed below) to be received by the end of the Sweepstakes Period. Winners’ names may be announced (subject to verification) on The Golf Channel.

6. General Conditions: Sweepstakes is subject to these Official Rules and all federal, provincial, state and local laws and regulations (collectively “Rules and Laws”). By entering Sweepstakes, entrants agree to all of the following: to be bound by these Rules and Laws, as well as the decisions of the Sweepstakes Administrator which are final and binding in all respects; to indemnify and hold harmless the Sweepstakes Entities from any and all liability resulting or arising from the Sweepstakes; to release all rights to bring any claim, action or proceeding against the Sweepstakes Entities; and to waive any rights to claim ambiguity with the Official Rules. Potential Grand Prize winner (his/her parent or legal guardian, if he/she is a resident of a jurisdiction that deems him/her to be a minor) may be required to sign, execute, have notarized, and return an Affidavit of Eligibility, (where legal) a Liability/Publicity Release and any other documentation that Sweepstakes Administrator and/or Sponsor requires (collectively “Affidavit/Release”), within three (3) days after receipt by potential winners or such potential winners will be disqualified and alternate potential winners may be selected. Grand Prize winner’s travel guest (his/her parent/legal guardian if he/she is a resident of a jurisdiction that deems him/her to be a minor) may be required to sign, execute, have notarized, and return a Liability Release, a Publicity Release and any other documentation that Sponsor requires within three (3) days after receipt by potential winner. By returning the completed Affidavit/Release, winners attest to the authenticity of their winning Entry. If winner is a resident from a jurisdiction that does not deem him/her to be of the age of majority prize will be awarded in the name of such minor’s parent/legal guardian. Once submitted, Entries become the sole property of Sponsor and will not be returned. Sponsor has the right to modify, use, assign, and/or dispose of such entries however it sees fit and for any purpose without any notice or compensation to entrant or any third party. Except where prohibited by law, entry into this Sweepstakes and/or winner’s (his/her parent/legal guardian, if winner is a resident of a jurisdiction that does not deem him/her to be of the age of majority) acceptance of prize constitutes permission for the Sweepstakes Entities to use winner’s name, address (city and state), photo, likeness, biographical information, statement and voice for advertising/publicity purposes worldwide and in all forms of media (including posting on web site), in perpetuity, without further notice to or compensation in connection with said Sweepstakes or the prize awarded. Entrant’s information, unless otherwise determined, may be shared with promotional partners for marketing purposes. Any personal information supplied by entrants to Sponsor or its affiliates will be subject to Sponsor’s privacy policy and terms of service posted at the Sponsor’s Web Site. Any person attempting to defraud or in any way tamper with this sweepstakes will be ineligible to win a prize and will be prosecuted to the full extent of the law. If, for any reason, the Sweepstakes is not capable of running as originally planned, Sponsor, at its sole discretion, reserves the right to cancel, suspend or modify any/all portion(s) thereof and conduct the drawing from all salvageable eligible Entries received, if any. In case of a dispute over the identity of an entrant who submitted a winning Entry, the owner of the email account associated with that entry will be deemed the entrant, at Sponsor’s sole discretion.

7. Disclaimers: Sweepstakes Entities are not responsible or liable for multiple entries that are entered by other than human means (such as by an automated computer program or any non-human mechanism, entity, or device), or for Entries that are late, destroyed, mutilated, forged, lost, stolen, misdirected, tampered with, incomplete, deleted, damaged, or otherwise not in compliance with these Official Rules and all such entries will be disqualified. By entering Sweepstakes, entrants agree and acknowledge that Sweepstakes Entities shall have no responsibility or liability for any property loss, damage, personal injury or death in connection with: participation in this Sweepstakes (including but not limited to, a particular Code or in the case of Canadian winners, disputes over answers to mathematical skill test questions); human error; acceptance/possession, use/misuse, and/or defects of the prizes awarded herein; incorrect or inaccurate transcription of Entry Information; any technical malfunctions of the telephone network, computer online system, computer dating mechanism, computer equipment, software, internet service provider; interruption or inability to access sweepstakes or Web Site or online service via the internet due to hardware or software compatibility problems; any damage to entrant’s (or any third person’s) computer and/or its contents related to or resulting from any part of this Sweepstakes; and/or any lost/delayed data transmissions, omissions, interruptions, defects or any other errors or malfunctions, even if caused by the negligence of one of the Sweepstakes Entities. Sweepstakes Entities are also not responsible for pre-emption/interruption/cancellation of show containing Code, promos, or elements of the Sweepstakes. Entrants further acknowledge that said Sweepstakes Entities have neither made nor are in any manner responsible or liable for any warranty, representation or guarantee, express or implied, in fact or in law, relative to any prize.

All requests must be received by January 2, 2004. Winners List may also be posted on Web Site.
